What is Androgen Deficiency in Aging Males (ADAM)?

Testosterone is an androgen hormone, and its levels naturally decrease in males as they age. For some, this deficiency doesn't drastically change how they feel. But others report decreases in energy, sexual drive, muscle mass and cognitive function. For them, an ADAM diagnosis can help them understand if testosterone replacement therapy (TRT) or another hormone optimization program will help them restore those vital lifestyle factors.

What causes low testosterone (low T) in males?

Most males experience decreased testosterone levels as they age. But other factors that correlate with low T levels include:

  • Having a BMI above 27
  • Smoking (or having ever been a smoker)
  • Having cardiovascular disease or high blood pressure
  • Having poorly managed type 2 diabetes
  • Having obstructive sleep apnea (OSA)
  • Having HIV or cancer
  • Being sedentary or exercising fewer than 75 minutes per week
What are some signs of low testosterone in men?

Testosterone is a type of hormone called an androgen. The brain has androgen receptors, and testosterone can cross the blood-brain barrier to stimulate them and affect cognitive function. So when testosterone levels are low, you may feel the effect in your body and brain.

Symptoms of low testosterone include:

  • Depression or low self-confidence
  • Difficulty concentrating
  • Disturbed sleep or sleep length
  • Declining muscle and bone mass, and increased body fat

How do you test testosterone levels with Optimize It?

With Optimize It, patients get blood work done at their local Quest Diagnostics on 2 separate occasions, a few days apart. These must be done before 11am—and at the same time on both days. Testosterone levels are highest in the morning, so this timing helps providers get the clearest picture of where natural T levels are consistently.

These tests also include essential factors such as electrolyte levels, lipids, optional vitamins that can affect hormone health and more. Then, patients meet with a provider via video to discuss the results of their lab work, symptoms and possible treatment options.

Treatment Options

What low testosterone treatments does Dr. B offer?

Right now, Optimize It offers two forms of Testosterone treatment—a Testosterone Cypionate intramuscular injection (Depo-Testosterone®) and Testosterone gel (Androgel®). Both of these safe, effective prescription medications providers call into your local pharmacy for pickup. You can pay with health insurance or out-of-pocket, using the drug discount card Dr. B will provide to help you secure the lowest medication cost in your area.

Will I need to take Testosterone treatment for life?

Yes, testosterone replacement therapy (TRT) is a lifelong medication. If you have Androgen Deficiency in Aging Males (ADAM), that means your body has stopped producing enough testosterone to keep you feeling great. TRT fills in that gap. Studies show that when patients with ADAM stop TRT, hormone levels revert to their pre-treatment baseline—or fall lower than they were before.

What is the best place to inject Testosterone?

The best place to inject intramuscular Testosterone treatment is in thick muscle tissue like the buttocks or upper thigh. But always follow your provider's particular instructions.

When does testosterone peak after injection?

With injectable Testosterone Cypionate, testosterone levels usually peak 2-3 days after the injection, then slowly go down over a week or two. With topical Testosterone, they peak within 2-4 hours. But after repeat administration, testosterone levels become steady within a few months of treatment.

Does Optimize It include supplements to increase testosterone?

Some vitamins and minerals affect testosterone production. With Optimize It, patients have the option to have some of these tested along with their required biomarkers.

During video consultations, providers may use blood work results or other observations to suggest supplements or dietary changes that may support T production. And all patients who complete lab work with Optimize It receive support materials (like emails and articles) that include guidance on how to prioritize micronutrient-dense foods for hormone health.

Side Effects & Risks

What are some common side effects of TRT?

Common mild side effects of testosterone replacement therapy may include nausea, vomiting, headache, changes in skin color, increased or decreased sexual interest, oily skin or acne and hair loss.

These symptoms are usually mild and may diminish over time. But request a callback from a provider if they get worse or do not go away

Does testosterone replacement therapy cause infertility?

Yes. Specifically, TRT can reduce sperm count. Adequate sperm count relies on a process where several hormones trigger each other to create or stop production. When testosterone levels rise above a certain point, hormone receptors in the brain signal the other hormones to halt production. Because TRT boosts testosterone levels quickly, studies suggest that treatment depletes sperm count within 2-3 months.

For many males, sperm count returns within 3-12 months after TRT is stopped—especially if other treatments are involved. But there's no guarantee this will happen. Some studies show that about 10% of patients do not regain their sperm count until somewhere in year two, and others never regain their fertility.

That's why organizations like the American Urological Association recommend not starting TRT if you are intending to start or grow your family in the near future. And if preserving future fertility is of any concern, make sure to discuss your options and some preventative steps with your primary care provider before starting TRT.

Does testosterone therapy cause high blood pressure?

Yes, the most extensive controlled testosterone trials to date show that TRT causes a slight increase in blood pressure. But these same studies did not correlate TRT with increased cardiovascular risk.

Still, if you have a cardiovascular condition or a family history of heart disease, we recommend discussing questions or concerns with your provider.

Can testosterone replacement therapy cause hair loss?
Yes, hair loss is a potential side effect of TRT. That's because about 10% of testosterone converts into dihydrotestosterone (DHT). This potent androgen (male hormone) is known to contribute to male androgenic alopecia by binding to receptors in hair follicles and disrupting their anagen growth phase. When TRT increases testosterone levels, DHT levels multiply. Hair loss can be a side effect for anyone on TRT, but those with male pattern baldness are at particular risk.

How much does online testosterone replacement therapy cost with Dr. B?

The cost of Testosterone treatment is determined by whether you pay for blood work and medication fees with insurance coverage or out of pocket. Then, the Optimize It consultation fee is $145. It includes a video appointment with a licensed provider and science-backed support materials packed with guidance on the various ways to optimize hormone health.

Why is TRT with Optimize It not offered in all states?

Because Testosterone medication is a controlled substance, laws determining how it can be prescribed differ from one state to another. Doctors must be additionally licensed in each individual state where they prescribe controlled substances. And in some states, patients can only get TRT if they meet with a provider in person. Dr. B strictly adheres to state and federal telehealth laws. So, while we're expanding our Optimize It treatment area, we cannot promise TRT treatment in every state.

Why are blood work fees not included with Optimize It?

Testosterone replacement therapy is a complex treatment that requires routine monitoring of specific biomarker levels in order to keep patients safe. Then, we offer patients the added option of monitoring vitamins and minerals that directly affect testosterone production. Finally, to qualify for treatment, we need two blood work results confirming low testosterone levels (since those levels can change daily). The first testosterone level test happens within a full biomarker panel, then testosterone alone is redrawn a few days later. For patients who start treatment, testosterone levels are checked every 3 months, and the full panel is rechecked yearly.

Because of this complexity, it's actually easier and more affordable for most patients to pay for their blood work with their health insurance coverage. For uninsured patients, we offer a discounted price for each set of required labs.

I'm a Shed It patient. If blood work fees aren't included with Optimize It, why are the consultation fees both $145?

Our mission at Dr. B is to offer safe, effective and comprehensive care—online! That means we take all medical complexities into consideration. While GLP-1 treatment is certainly complex, hormone optimization treatments (like testosterone replacement therapy) require additional time from medical doctors and more comprehensive blood work from patients. Testosterone is also a controlled substance, and GLP-1 medications are not. That means we can't price these treatments equally.
